Our practice will take all reasonable steps to insure that the minimum necessary amount of information is disclosed to accomplish practice operations, obtain payment for your services, and render treatment to you. This includes sending claims and records to your insurance company to obtain payment, maintaining dental office notes, discussions with insurance companies to obtain payment, discussion with collection agencies, if necessary, and discussion with other healhcare providers in the course of treatment.
Your entire dental record will never be released to anyone, unless specifically authorized by you in writing. You have a right to restrict to whom you will allow a portion or all of your records released. Your records may, however, be released without an authorization in the course of legal investigations by state or federal agencies. If you need to restrict to whom your records are released, please contact our Office Coordinator.
You have a right to inspect your dental records with staff present, with reasonable notice to me. You have a right to ask that your dental records be amended. They cannot be erased or changed. Your dentist is not obligated to comply with the request to amend the record. Your request must be in writing.
You have the right to know where and when your personal health information is sent. Our staff has been trained in the policies and procedures concerning the release of protected health information.
